An A and D Medical upper arm blood pressure monitor from the UA-767 range, supplied new with its accessories including the mains adapter. The 767 is the automatic oscillometric monitor that has been a fixture in GP practices, care homes and home monitoring for years, and it is still specified where a reading has to be defensible rather than merely convenient.

Measurement runs across 20 to 280 mmHg with a pulse range of 40 to 200 beats per minute. Pressure accuracy is held within 3 mmHg or 2 per cent, whichever is the greater, and pulse within 5 per cent. The cuff inflates on a micropump and deflates through an exhaust valve without the operator touching anything, which removes the two sources of error that matter most in a busy clinic: inconsistent inflation and inconsistent bleed rate. Irregular heartbeat is flagged on the display while still giving the reading rather than refusing to report.

Cuff size is the single most common cause of a wrong blood pressure reading, and A and D publish three for this range: small at 16 to 24 cm arm circumference, reference UA-279, medium at 24 to 36 cm, reference UA-280, and large at 36 to 45 cm, reference UA-281. A cuff that is too small reads high and one that is too large reads low, so the arm is measured and the cuff chosen rather than assumed. Only the cuff supplied with this monitor is included.

The display shows systolic, diastolic and pulse together, with 16 mm digits for the pressures and 10 mm for the pulse, which matters for an older patient reading their own result. The unit runs on four AA alkaline cells, around six months at one measurement a day, or from the mains adapter, reference TB-233 and sold in the United Kingdom as AX-TB233BS. For a monitor used several times a day on a practice desk the adapter is what stops it eating batteries, and it is part of this sale.

Points to note before ordering

  • Which 767 variant. The UA-767 range includes variants that differ on memory capacity, number of users, atrial fibrillation screening and Bluetooth. Elarix Medical confirms the exact model and which of those features this unit carries in writing before dispatch, rather than stating them from the range name.
  • Cuff size decides accuracy. A cuff that is too small reads high and one that is too large reads low. Measure the arm circumference and match it to the 16 to 24 cm, 24 to 36 cm or 36 to 45 cm cuff. Only the cuff supplied with this monitor is included.
  • Screening, not diagnosis. An automatic monitor supports measurement. A diagnosis of hypertension, and any change to medication, remains a matter for a clinician, and an irregular heartbeat flag is a prompt to seek advice rather than a diagnosis of atrial fibrillation.
  • Technique. A reading is taken seated and rested, with the arm supported at heart level and the cuff on bare skin. Readings taken over clothing, with the arm hanging, or straight after exertion are not comparable.
  • Accessory set. The accessories supplied with this unit are itemised in writing before dispatch.
