A complete bilevel setup built around a pre-owned DreamStation BiPAP AVAPS in its humidified configuration, supplied with a new DreamWear mask starter pack, a new HT15 heated tube, a new water chamber and a CPAP cleaner. Everything needed to begin therapy is in the box except the prescription and the titration, which come from the responsible clinician.

The device is a non-invasive ventilator rather than a sleep apnoea machine. It runs CPAP, S, S/T, T and PC modes from 4 to 30 cm H2O in 0.5 cm steps, and adds Average Volume Assured Pressure Support, which moves the inspiratory pressure breath by breath to hold a target tidal volume between 200 and 1500 ml. A CPAP device holds one pressure to splint an airway open; AVAPS measures the volume actually achieved and adjusts to reach the target the prescriber set. It is the therapy used where ventilation is failing rather than where an airway is collapsing, which in practice means obesity hypoventilation, chronic obstructive pulmonary disease with carbon dioxide retention, neuromuscular disease and chest wall disease.

The three accessories are chosen to work together rather than gathered at random. The humidified version of the device is supplied, so the heated tube has something to do: the HT15 is a 15 mm heated tube about 1.8 m long that holds the air above its dew point from machine to mask, which is what lets humidity be run at the level the airway needs instead of the level at which the hose starts raining water into the face. The water chamber is the consumable that makes the humidifier work and is supplied new. The CPAP cleaner covers the routine the mask and chamber need between uses.

The DreamWear mask is worth a word of its own. Its frame carries the air over the top of the head and the tube connects at the crown, so there is no hose lying across the face and nothing in the line of sight. For anyone who sleeps on their side, turns during the night, or finds a conventional mask closed in, that single design decision is usually the difference between tolerating therapy and abandoning it. The starter pack supplies the frame, headgear, elbow swivel, fabric wraps and a full set of cushion sizes, so the fit is worked out at home rather than through a second order.

Points to note before ordering

This device family was the subject of a field safety notice. In June 2021 Philips Respironics issued a field safety notice covering certain sleep therapy devices and ventilators over degradation of the polyester based polyurethane sound abatement foam fitted inside them, and the first generation DreamStation range falls within it. Elarix Medical checks this unit's serial number against the Philips remediation record, states in writing whether it has been remediated and what was done, and does not supply a unit for patient use where that cannot be established.
Non-invasive ventilation, supplied against a prescription. AVAPS is a prescribed therapy for respiratory failure, not a product chosen from a shelf. Target volume, pressure limits and backup rate are set and titrated by the responsible clinician, and the bundle is supplied to a healthcare provider or against a prescription rather than direct to a member of the public.
The device is pre-owned, the accessories are new. Mask, heated tube, water chamber and cleaner are supplied new and unused. The device carries marks consistent with use and is decontaminated to local policy, electrically safety tested, and function checked against a test lung with delivered pressure verified before dispatch. Elarix Medical states the running hours on request.
Consumable service life. Mask cushions are generally changed every one to three months, headgear every three to six, the complete mask every six to nine, and tubing every three to six, with the instructions for use supplied with each item governing. Air filters are not part of this bundle and are ordered as ongoing stock.
