Smiths Medical Level 1 System 1000 H-1000 Fast Flow Blood and Fluid Warmer, Pre-owned

The Level 1 H-1000 is a fast flow blood and fluid warmer, built for rapid infusion rather than for routine drip line warming. Fluid passes through a disposable countercurrent heat exchanger surrounded by a circulating water bath held at around 42 degrees Celsius, and reaches the patient near 40 degrees Celsius. Side poles carry pressure infusers or pumps so the whole arrangement stands as one unit at the head of the table.

The clinical case for a device like this is hypothermia, and specifically hypothermia caused by the treatment rather than by the injury. In major haemorrhage a patient receives refrigerated blood at speed, and infusing cold fluid fast drops core temperature quickly. Hypothermia then impairs clotting and worsens acidosis, which is the spiral that makes bleeding harder to stop. Warming the fluid at the rates actually being used is the point: a device that warms adequately at 50 ml per minute is of little help when the transfusion is running several times faster.

Countercurrent exchange is what makes that possible. The water flows along the heat exchanger in the opposite direction to the fluid, so a temperature difference is maintained along the whole length rather than being used up in the first few centimetres. That is why the device holds its delivered temperature as flow rises, and it is the reason a water bath warmer outperforms a dry plate at high flow.

Air is the other problem at speed, and the set carries a 170 micron screen filter with a gas vent on luer lock connections, changed every three hours or sooner if it clogs. The unit alarms on three conditions: add water, check disposables and water over temperature. It runs on distilled water in the bath. Supplied pre-owned.

Features

  • Built for rapid infusion rather than routine drip line warming.
  • Countercurrent heat exchange, holding the delivered temperature as flow rises.
  • Circulating water bath at around 42 degrees Celsius, delivering fluid near 40 degrees Celsius.
  • Side poles carrying pressure infusers, syringe pumps or infusion pumps, so the arrangement stands as one unit.
  • 170 micron screen filter with a gas vent on luer lock connections.
  • Alarms on add water, check disposables and water over temperature.
  • Runs on distilled water in the bath.
  • Addresses the hypothermia that rapid transfusion itself causes, rather than the hypothermia the patient arrived with.

Points to note before ordering

  • The device does nothing without its disposable set. The heat exchanger is part of the single use set, not part of the machine, so the running cost and the availability of sets matter more than the purchase price. Smiths Medical was acquired by ICU Medical in 2022, so Elarix Medical confirms the current set references and a supply route in writing before dispatch rather than leaving a buyer with a warmer and nothing to put through it.
  • The heat exchanger is anodised aluminium, and that has been the subject of published debate. Aluminium release from aluminium based fluid warmers has been examined in the anaesthetic literature, including work in Anaesthesia on this specific device and a reply and counter-letter in the same journal, and further work in BMC Anesthesiology. The position is contested rather than settled. Elarix Medical makes no claim either way, and states the material fact so that a buyer's own clinical governance can take a view, particularly where neonates or patients with renal impairment are concerned.
  • Water bath maintenance is the routine that keeps it working. The bath takes distilled water, not tap water, and the add water and water over temperature alarms are the ones that take the device out of service. Elarix Medical confirms the bath is sound, the heater and circulation function, and the alarms sound before dispatch.
  • This is not a Hotline. The Level 1 Hotline is a different device for lower flows and warms at the patient end. The H-1000 is the fast flow machine for major haemorrhage, and the two are not interchangeable either clinically or in their consumables.
  • Pre-owned condition and commissioning. Marks consistent with theatre use. To be decontaminated to local policy, electrically safety tested, and function checked with delivered fluid temperature verified against an independent thermometer across the flow range before clinical use. Elarix Medical photographs the actual unit on request.
